New Passport Requirement:
If you have never applied for a passport, or if it is more than 15 years from the issue date of
your existing passport, the following procedure applies to you:
Prepare these documents:
1. A certified copy of birth certificate. Hospital records are NOT acceptable.
2. Two recent taken passport-type identical photos.
4. A check, cashier check or money order for $170 ($140 minors) payable to
"US Department of state"
Note: Effectice 02/01/2008 applicants may also apply for "Passport Card". If applying at the same time with your passport, just mark the corresponding box on application and add $30
to the filling fees. You may also use renewal application to apply for "Passport Card" separately.
In this case, filling fee for passport card is $90 ($75 for minors) plus the $25 local processing
fee and the service fee depending on the type of process times requested.
5. Include a Photocopy of current (valid) State ID or driver's license.
6. A copy of your proof of travel such as travel itinerary, airline ticket or business letter.
7. If you have had a name change, include a certified copy of marriage certificate, or certified
copy of court order/decree.
8. Complete and sign authorization form. This form allows us to pick up your new passport from
the passport agency and must included in the same-package with your application.
Passport Renewal:

1. Your expired or about to expire passport.
2. Two recent taken passport-type identical photos.
